CONFIDENTIALITY POLICY

As a registered nurse operating under AHPRA and the Nursing and Midwifery Board of Australia (NMBA), I am committed to maintaining the highest standards of privacy and confidentiality.

All client information—including personal details, medical history, treatment records, and conversations—is kept strictly confidential and will only be shared with others involved in your care, with your consent, or when required by law (e.g., mandatory reporting).

Your records are stored securely, and access is limited to authorised personnel only. I do not share any client information on social media or public platforms.

This policy complies with the Privacy Act 1988 (Cth) and all relevant professional and legal standards. Breaches of confidentiality are taken seriously and may be reported to AHPRA.

All IV infusion services provided are intended solely for wellness and general health support. They are not a substitute for professional medical advice, diagnosis, or treatment. We strongly advise that you consult with your own licensed physician or healthcare provider before beginning any IV infusion therapy.

By receiving IV infusion services from us, you acknowledge and agree that:

You assume full responsibility for consulting a qualified healthcare professional regarding any medical conditions or concerns you may have.

MOBILE VISITS:

0 to 15 mins – $0
15 to 30 mins – $20
30 – 45 min – $40
45-60 min – $60 (minimum spend $229)
Over 1 hour $75 +

DEPOSITS & CANCELLATION POLICY:

A 30% deposit is required to secure all bookings. Should you need to cancel or reschedule, please provide at least 24 hours’ notice to avoid losing your deposit. In the event of a no-show or arriving more than 15 minutes late, the deposit will also be forfeited.

SCRIPTING:

For your treatment, our doctor will provide a personalized script for all vitamins, nutrients, minerals, and amino acids. This script comes with a fee of $50 and will be valid for 12 months, exclusively with Haven IV Infusions.

STAFF TREATMENT:

We pride ourselves on creating a welcoming and respectful environment. In return, we kindly ask that all clients treat our staff with the same courtesy. Inappropriate or disrespectful behavior will not be tolerated.

MANDATORY DISCLOSURE/CONSENT FORM:

Your health and safety are our top priority. Before we confirm your appointment, you will need to complete a mandatory medical declaration and waiver consent form. Please make sure to inform us of any medical conditions, allergies, medications, or contraindications prior to your appointment. This will allow us to ensure that your treatment is both safe and effective. If any important details are withheld, we may need to cancel the appointment, and your deposit will be forfeited. By signing the waiver, you acknowledge and accept all risks associated with vitamin therapy, whether administered intravenously, intramuscularly, or subcutaneously.
Haven IV Infusions reserves the right to modify or terminate any appointment at its discretion to maintain the highest standards of care.

SUBSCRIPTIONS:

Subscribers must be 18 years or older. Drips do not roll over and must be used within the billing cycle. Each plan includes any IV drip up to the value of the Limitless drip. A minimum commitment of 3 months applies, with 30 days’ notice required for cancellations. A yearly prescription fee of $50 applies. All clients must complete a health screening and provide informed consent prior to treatment. Mobile fees apply.
